Intake and Interview Forms: In most cases, you should pick up a packet to forms to complete prior to your scheduled appointment. Information about packet pick-up will be provided at the time you make your appointment. If you are not able to pick up your packet, you can download most of the forms you need and print them out at home. Be sure to fill them out before arriving at your appointment.

Economic Impact Payment Worksheet - Required for ALL
Itemized Deductions Worksheet - Required if you want to itemize deductions on either your federal or state returns OR if you want to claim the addition to the Standard Deduction for charitable contributions.
Worksheet for Self-Employed Business Income and Expenses - Required if you are self-employed.
Worksheet for Self-Employed COVID Sick and Family Leave - Required for self-employed persons who wish to claim the credit for days of work missed due to COVID-related reasons. DOES NOT APPLY TO PERSONS WHO ARE NOT SELF-EMPLOYED OR WHO HAD SICK LEAVE PAID BY AN EMPLOYER.
