Intake and Interview Forms:

• Appointment sites: In most cases, you should pick up a packet to forms to complete prior to your scheduled appointment. Information about packet pick-up will be provided at the time you make your appointment. If you are not able to pick up your packet, you can download most of the forms you need and print them out at home. Be sure to fill them out before arriving at your appointment.

• Walk-in sites: You will be given blank forms to complete when you sign in at the site, although you can complete forms ahead of time if you wish.

• Intake and Interview Booklet - Required for ALL

• Hawaii Supplemental Intake Form - Required for ALL

• Itemized Deductions Worksheet - Required if you want to itemize deductions on either your federal or state returns.

• Worksheet for Self-Employed Business Income and Expenses - Required if you are self-employed.

• Education Credits Worksheet - Required for those with expenses for higher education (post-high school) wishing to claim either the American Opportunity Credit or the Lifetime Learning Credit.
